Cornerstone Support Services is licensed to provide long-term services to adults over the age of eighteen with a diagnosis of intellectual disability or a co-occurring diagnosis of developmental disability and mental health issues.

RESIDENTIAL GROUP HOMES GROUP HOME SERVICES
The Group Home model is designed for 3-4 people to share a home operated by Cornerstone. Direct Support Professionals provide a structured program of care 24 hours a day, 7 days a week.
SUPPORTED LIVING
The Supported Living Model is designed for 1-3 individuals to share an apartment operated by Cornerstone. Individuals enjoy independence while receiving supports from Direct Support Professionals as needed throughout the day.
COMMUNITY INTEGRATION
A quality of life enhancing add-on service that we have incorporated into our regular Residential and Supported Living programs that encourages people to interact and engage in their respective communities.
Cornerstone Team Members encourage people utilizing our programs to not just be present in the community, but participate and become an active member of their community.
This is not a separate service but rather something we felt should be part of our regular Residential and Supported Living Models.
ADMISSION CRITERIA
Cornerstone Support Services is licensed to provide long-term services to adults over the age of eighteen with a diagnosis of intellectual disability or a co-occurring diagnosis of developmental disability and mental health issues.

Intellectual Disability, as defined by the American Association of Intellectual Disabilities is a disability characterized by significant limitations both in intellectual functioning and in adaptive behavior, which covers many everyday social and practical skills. This disability originates before the age of 18.
Developmental Disability as defined by Developmental Disabilities Assistance and Bill of Rights Act originates at birth or during childhood, is expected to continue indefinitely, and substantially restricts the individual’s functioning in several major life activities; is attributable to a mental or physical impairment or a combination of mental and physical impairments; is manifested before the person attains age 22; results in substantial functional limitations in three or more of the following areas of major life activity: self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living, and economic self-sufficiency.
Compatibility of services and chemistry of household are also considered part of admission approval.
All of the following criteria are necessary for admission:
- The individual must be 18 years of age or older.
- The individual has been assessed and diagnosed by a trained professional.
- The primary diagnosis must be Developmental Disability / Intellectual Disability, with or without co-occurring issues.
- The services provided by Cornerstone have been determined to be appropriate for the individual.
